Waiver and Participation Agreement
Liability Waiver and Release
I understand that participation in Sensory Time by Fun Bins involves hands-on play activities that may include small objects, loose materials, tools, movement, and age-appropriate equipment. I acknowledge that participation carries inherent risks, including but not limited to minor injury, allergic reactions, or accidents.
I knowingly and voluntarily assume all such risks, both known and unknown, even if arising from the ordinary negligence of the RELEASEES (but not gross negligence), and I assume full responsibility for my child’s participation.
I, for myself and on behalf of my child, heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Fun Bins, LLC, its owners, officers, staff, contractors, volunteers, host locations, sponsors, and property owners (collectively, the “RELEASEES”) from any and all claims, demands, losses, or causes of action for injury, disability, death, or loss or damage to person or property, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT, AND AGREE TO IT FREELY AND VOLUNTARILY.
Parent / Guardian Certification (For Participants Under 18)
I certify that I am the parent or legal guardian of the minor participant and have the legal authority to execute this agreement on the minor’s behalf. I agree that this waiver applies to all Sensory Time by Fun Bins sessions unless revoked in writing.
Health & Safety Acknowledgement
I understand that Sensory Time by Fun Bins is a recreational sensory play experience and is not a preschool, daycare, therapy, or medical program.
I acknowledge that participation may involve sensory materials, small objects, and movement. I confirm that my child is able to participate safely with my direct supervision.
I agree to inform Fun Bins, LLC of any known allergies, sensitivities, or relevant medical conditions prior to participation. I understand that while reasonable precautions are taken, an allergen-free environment cannot be guaranteed.
I agree to remain present and actively supervise my child at all times during the session.
Photo & Video Consent
Photographs or short video recordings may be taken during Sensory Time sessions for marketing, educational, or promotional purposes, including website, social media, and printed materials.
No names or identifying personal information will be shared.
